    Mercy Care Advantage (HMO SNP)

    This is a health maintenance organization special needs plan (HMO SNP). It provides Medicare benefits to people who qualify for Medicaid, as well as Medicare Parts A and B.

    Service area:

    •  Arizona Health Care Cost Containment System (AHCCCS) Complete Care Regional Behavioral Health Agreement (ACC-RBHA): Gila, Maricopa and Pinal counties

    •  AHCCCS Arizona Long Term Care System (ALTCS): Gila, Maricopa, Pima and Pinal counties 

    •  Division of Developmental Disabilities (DDD): Statewide

    One plan, one card

    Are you a member of both Mercy Care Advantage and Mercy Care Medicaid (AHCCCS)? If so, you only need to show one member ID card at visits to your health care providers. Here’s how it works:

    1. Show the provider your Mercy Care Advantage card. It has three logos on it:

      • Mercy Care Advantage
      • Mercy Care
      • AHCCCS
    2. Need another ID? Show your Arizona driver’s license or state ID card.
    3. You don't need to show your AHCCCS card as well.
       

    Do you get the Medicaid part of your coverage from a plan other than Mercy Care? If so, you’ll need to show both cards at visits to your health care providers:

    • Your Mercy Care Advantage ID card 
    • The ID card for your other Medicaid coverage (not Mercy Care)

    This allows your health care providers to submit claims to both your health plans.

    Your Mercy Care Advantage ID card helps protect your identity. Here’s how it works:

    • AHCCCS uses the picture on your driver’s license or state ID to confirm who you are. 
    • Then, they add your picture to the online tool that providers use to check your coverage. 
    • Your providers can see your picture and confirm your benefits.
